Before Your Consultation
Be Ready for Your First Appointment
To help us provide the best care possible at your dental consultation, please bring the following—and don’t hesitate to call if you have questions ahead of time.
- Dental Insurance Information: If you have dental insurance, please bring your card and plan details.
- State-issued ID: Required by most insurance providers.
- Medical History: Let us know about any health conditions that may impact your dental care, such as diabetes, high blood pressure, osteoporosis, or artificial joints or heart valves.
- Medications: Please share a list of any medications you’re currently taking, especially blood thinners or bone-strengthening medications like bisphosphonates.
- Pre-Medication Needs: If you’ve been advised to take antibiotics before your dental visit, please let us know in advance.
- Patients Under 18: A parent or legal guardian must accompany any minor to their dental visit.
We’re here to make your visit seamless—feel free to call our office ahead of time if you’re unsure about what to bring.

Prism Dental Membership
Membership Plan
No Insurance? No Problem.
For patients without dental insurance, we offer a simple, affordable in-house membership plan that gives you access to preventive care, emergency visits, and discounts on treatment—with no deductibles, no waiting periods, and no confusing paperwork.
What’s Included:
- 2–4 professional cleanings per year (based on membership type)
- 2 comprehensive dental exams
- All necessary X-rays
- 2–4 in-office fluoride treatments
- 2 emergency exams
- 20% off all additional treatment
Patients who need scaling and root planing receive 20% off their initial procedure and pay only 50% of the regular cleaning fees for periodontal maintenance—making two of those cleanings essentially free.
Membership Pricing
- Individual Plan: $200/year
- Spouse or Family Member Add-On: +$150/year per person
- Child Plan (12 & Under): $100/year

Dental visits don’t have to be stressful. We’ve created a space where you can actually relax. Our Comfort Menu includes:
- Soft blankets and pillows
- Noise-cancelling headphones
- Streaming video options (Netflix, Spotify, etc.)
- Aromatherapy options
- Friendly, low-pressure conversations (no lectures!)
Need a little extra help relaxing? We offer oral sedation and nitrous oxide for those who experience dental anxiety. Just ask—we’ll walk you through what to expect.
